The ECFMG/USMLE Registration process

Feb. 2 - I applied through IWA for my USMLE ID # (no charge) about a week before. But I started reading FA February. (I planned my steps in January)
 ~~~~~~~~~~~
Feb 19 - I received an email from ECFMG about the clinical rotation part of my online application, because I was hesitant at answering since I have forgotten them.
here's the answer:
February 19, 2010
Dear Doctor,
Your permanent USMLE®/ECFMG® applicant number is XXXXXXXXXX.  Please be sure to quote this number on all application forms, payments, credentials and correspondence sent to ECFMG in the future.
Please be advised the clinical clerkship section of the Interactive Web Application (IWA) is not an important item.  Please complete the question to the best of your knowledge.  The information you provide here will not affect your eligibility to take the examinations nor will we report it to any third parties, or verify it with your medical school.
Sincerely,
JAY
 My email:
To: Info
Subject: first time USMLE application
To Whom It May Concern:
I am a doctor from the Philippines who graduated from XXXXXXXXXX College of Medicine in the year 1996. I am currently trying to complete the online part of USMLE application, but I stopped at the point where it asks for clinical clerkship. I had my clinical clerkship at various affiliated hospitals aside from my medical school's own hospital from 1995-1996 during that time, and I do not have documentation of those no do I remember the names of supervising physicians in each discipline. I would like to know if I need to make the extra effort to contact my school (I am now a permanent resident of the USA) to see if they could provide me the necessary information, or will there be a  later step in your coordination with them that will verify such information as well, as part of my application process? I do not mind if I need to contact them, but I do worry if my documents pertaining to that information remain intact after a recent severe flood that hit the Philippines (Typhoon Ondoy, or internationally known as Ketana). If so, then I would not be able to complete the online part of my application process and will never have a chance at becoming licensed as a doctor here in the USA.
Other than the clinical clerkship information, I do have with me my diploma and transcript of records, and I am sure you will be able to verify those information directly from my medical school.
I would appreciate your enlightening reply.
Thank you,
Dear Doctor:
The processing of your USMLE Step 1 application is complete. You have been registered for theXXXXXXXXXXX eligibility period in the United States and Canada testing region. ** Please see BELOW for details on the option to submit a request to have the exam results for this Step 1 withheld from your medical school**.
The eligibility period assigned to you for this exam may be different from the one you selected on your application for one of the following reasons:
   - If you selected an eligibility period that has already begun and your application was not processed in time to assign the eligibility period you selected, you have been assigned the next eligibility period.
   - USMLE policy states that you may not take the same Step more than four times in any 12-month period.  If you have already taken this exam and the eligibility period you selected on your application does not comply with these requirements, you have been assigned to the first eligibility period that does meet these requirements.
The National Board of Medical Examiners(r) (NBME(r)) will issue your permit, which contains information on contacting Prometric to schedule a testing appointment. (If your assigned eligibility period begins more than six months from now, your permit will be issued approximately six months before the beginning of your assigned eligibility period.) Only electronic permits will be generated for Step 1/Step 2 CK; applicants will access their electronic permits using ECFMG's Interactive Web Application (IWA).  You will not receive a paper permit by postal mail.  In about one week you will receive another email from ECFMG to notify you that your permit is available.  This message will include instructions for accessing the electronic permit using IWA. Although your permit will be issued by NBME, you should contact ECFMG by email at info@ecfmg.org or by phone or fax if you have questions or concerns about this document. You must print a copy of the electronic permit and bring it to the test center on your exam date. Your permit is a very important document, and you should be careful not to lose it. You cannot schedule your testing appointment or take the exam without it. If you lose your permit, you may print another one by using IWA.  Your permit will be available on IWA until the LAST day of your eligibility period.  However, ECFMG recommends that you print your permit as soon as it is available to you and keep it in a safe place. Upon accessing your permit, you should check that your name as it appears on the permit is spelled correctly.  If the name on your permit is incorrect, you must contact ECFMG immediately.  Name changes or corrections to your name must be received and processed by ECFMG no later than seven business days prior to your scheduled testing appointment.
If you cannot take a Step during your assigned three-month eligibility period, you may request an extension. There is a fee for this service. You may request an extension on-line using ECFMG's Interactive Web Application (IWA) or by submitting to ECFMG a completed Request to Extend a USMLE Step 1 / Step 2 CK Eligibility Period (Form 176). For information, access IWA or FORM 176 on the ECFMG website.
For current information on test centers in your testing region, visit the Prometric website at www.prometric.com or contact Prometric. Sample test materials for all Steps and Step Components are available on the USMLE website at www.usmle.org
Please refer to the ECFMG Information Booklet and USMLE Bulletin of Information FOR detailed information on scheduling your testing appointment and taking the exam. BOTH publications are available on the ECFMG website.
Instructions on how to withhold your exam results from your medical school
ACCESS the IWA home page at www.ecfmg.org. SELECT the exam administration for which you want to withhold your exam results from your medical school under "Request to Withhold Exam Results from Medical School".  Be sure you read and understand the conditions before you authorize ECFMG to withhold your exam results.
Sincerely,
Registration and Credentials Services
Mar 17 - I received this email:
Your STEP 1 SCHEDULING PERMIT IS NOW AVAILABLE on the ECFMG Interactive Web Application (IWA) website.
YOU MUST PRINT YOUR SCHEDULING PERMIT AND BRING IT WITH YOU TO THE TEST CENTER ON YOUR SCHEDULED DATE.  You will not be able to take the test if you do not bring your Scheduling Permit to the test center.  Note: You will NOT receive an orange Scheduling Permit for this exam in the mail.
To access your Scheduling Permit, go to IWA at https://iwa2.ecfmg.org, login, and select "Print/Reprint your USMLE Step 1 Scheduling Permit."  Your permit will open in a new browser window.  Use your browser's "Print" function to print the permit.
If you have any questions, contact ECFMG Applicant Information Services at info@ecfmg.org or call (215) 386-5900.
Sincerely,
ECFMG
Mar 17 (same day) I scheduled my testing.
Another USMLE personal documentation...
I made a half-baked decision around December 2008 to finally pursue the medical career. So I purchased the expensive Kaplan home study reviewer (4 books in a set) that I got from amazon.com at a whopping $400++ price. I told myself, the price seals my commitment. I started working around March as an ED RN, then I got an email in another hospital asking if I still considered getting into OB-Gyn/Maternal ward (I applied in spring of that year). I grabbed the opportunity because then I was planning of leaving the ED (too much adrenaline for me!).
I was back to Rehab by January 2009, started in OB-Gyn, and forgot about my USMLE plans as they got to the background while I was learning new stuff in the ED then and OB-Gyn now. By October, hubby got into an accident and I chose to stay more at home. I let go of my OB-Gyn and continued working part-time in Rehab. My USMLE interest resurfaced. By December (after all the Holidays hullabaloo, I decided to continue pursuing Medicine, also for better future, and just because I wanted a more pro-active patient management position. So I downloaded the ECFMG and USMLE handbooks around January and read them to learn more how to proceed. I posted in forums, and asked around.
I got FA and checked out the book lists at the back. Asked in forums, and decided on my reviewers.
Goodluck to me, as it has been more than 10 years when I graduated (1996, to be specific). Many things medical have changed.
